States do not appear in pull down

Hi,

When a customer fills in the “Billing Address” and “Shipping Address” in the Checkout process, no States appear for the “Billing Address”, but strangely, they do perfectly appear when filling out the “Shipping Address”.

See attached image with both “Billing Address” and “Shipping Address” pull down menus selected.

What could be wrong?
