How to Add fixed cost - independently from quantity?

from frontend it's possibile to choose a type of printing. so, needs to add the quantity, but I need to add the fixed setup cost that is independent of the amount set?

Example:

Laser print: 12 eur
Q.ty: 500
Setup: 10
Total: (500x12) + 10
------------

Normal print: 7 eur
Q.ty: 2000
Setup: 8
Total: (2000x7) + 8
