Calculate for price

Hello,

I have bought Calculate for price, and since I have installed it, no shipping options can be seen on the frontend. I tried to solve the problem, and found that configform.php (in the downloaded .zip) is empty - is it normal?

I HAVE ALREADY downloaded it. And I have just downloaded it again a minute ago - configform.php is an empty file. If this can cause my problem, please, send me a correct configform.php file, or a correct link where I can download it from.

At this moment it looks like I paid for nothing, bought a product which does not work. Please, help to solve this problem.

I asked you questions, because you are supposed to be the support, who answers the questions. If I know something, I wouldn't ask.

So, once more:
1. I had a working webshop
2. I have bought Calculate for price
3. I have installed Calculate for price
4. I have set different shipping costs for different prices
5. Since then, no shipping options can be seen in the frontend. It means, that "next" button is inactive, so that the customer can not neither select a shipping option nor continue/finish his shopping process.

Could you help me, what the problem is?

1. Set price for country (option / shipping prices - select countries )
2. Set shipping for payment
